The annual health assessment involves:

Any patient aged 75 years or older who lives at home (not in a residential aged care facility) with a valid Medicare card is eligible. The assessment is available once per year.

The assessment covers: full patient history, medication review, continence, immunisation status (flu, tetanus, pneumococcus), physical function and fall risk, cognitive and mood assessment, social support, blood pressure and pulse, oral health, and nutrition.
